No form. IVP's own FAQ tells founders to "get started by emailing the investor(s) experienced in partnering with businesses like yours", and every team bio on ivp.com publishes that person's direct email address. So the intended route is a cold email to one named investor, not a warm intro.
Worth knowing. Two things in their FAQ change the approach. First, they explicitly say do not wait until you are raising to reach out, and offer feedback on trajectory as a reason to make contact. Second, the bar is stated in revenue terms rather than round names: they partner with about a dozen founders a year who are "ready to scale from millions to hundreds of millions in revenue". If you cannot describe yourself that way, the email will not land regardless of who you send it to.
