Gentiva, a dedicated hospice, palliative, and personal care company driving improved access, equity, and quality of care for patients, and ProMedica, an integrated healthcare organization providing, in part, hospice and home care services through its Heartland brand, today announced that they have signed a definitive agreement under which Gentiva will acquire Heartland hospice and home care agencies/locations from ProMedica.
